I always use an off camera strobe in one hand to maneuver the light while taking the picture with the other. Also, drastically slow down the shutter speed for some excellent blur to accompany the initial exposure a good flash grants you + zoom in/out/move the camera to make light trails. I don't do much of the zooming in or out due to no free hand. Maybe if I had a flash mob of assistants...
